Pop Warner Requires a physical dated the same calendar year for the current Tackle and Flag season (The same physical can be used for both activities). Pop Warner also requires that the physical be documented on the above "Physical Fitness and Medical History Form". The Pacifica Tigersharks recommend that you see your own private doctor, but if you are unable, please call Dr. Chan at 650-756-9003 for an appointment (usually same day). The cost is $20.00.
